GolfRx Master Client Agreement & Terms of Use

Welcome to GolfRx. By signing up for and using any GolfRx services, including therapy coordination, wellness resources, and golf-related programs, you agree to the terms outlined in this Master Client Agreement and Terms of Use. This Agreement governs your relationship with GolfRx (“Company,” “we,” “us,” or “our”) and applies to all services provided through our platform.
1. Service Overview
GolfRx provides a unique blend of mental wellness tools, therapist support, and golf-based therapeutic programs. Your plan features, benefits, and reimbursement levels are based on the subscription you selected during sign-up. These may include:

Therapist access and sessions

Reimbursed golf activities

Personalized interventions or assignments

Secure messaging or check-ins
You can review your selected plan details by referring to your confirmation email or member dashboard.
2. Client Responsibilities
By using our services, you agree to:

Provide accurate, up-to-date information upon sign-up.

Maintain a valid payment method on file for recurring billing.

Use our services in a lawful and respectful manner.

Not share your membership or access credentials with others.
You also acknowledge that therapeutic services are provided by independently contracted licensed therapists, and GolfRx serves solely as a platform for coordination and reimbursement.
3. Health & Eligibility
GolfRx is not a substitute for medical or psychiatric care. If you are experiencing a crisis or medical emergency, call 911 or seek immediate medical attention.
You may only participate in reimbursable golf activities if you are physically able and cleared by a medical professional to engage in physical activity. We reserve the right to request medical clearance documentation if needed.
4. Payments & Billing
You authorize GolfRx to charge your selected payment method for the recurring monthly fee associated with your chosen plan. All charges are billed in advance on a monthly basis and are non-refundable unless otherwise stated.
If your payment method fails or expires, we may suspend access until the account is brought current.
5. Reimbursements
Reimbursements for eligible golf activities are issued monthly based on your plan’s reimbursement tier. Reimbursement timelines may vary but are typically processed within 15 business days of receipt approval.
6. Therapist Relationship
Therapists on the GolfRx platform are independent contractors. While we verify licensure and conduct due diligence, we do not control the clinical judgment or methods of therapists. All clinical responsibility lies with the individual therapist.
7. Privacy & Data
We take your privacy seriously. Your information is protected under HIPAA where applicable. Please review our full Privacy Policy for details on how we collect, store, and use your information.
8. Cancellation & Changes
You may cancel or change your plan at any time by emailing support@gogolfrx.com. Changes will take effect at the end of your current billing cycle. Early termination of annual plans may result in forfeiture of unused months or applicable discounts.
9. Limitation of Liability
GolfRx is not liable for any injuries, losses, or damages resulting from your participation in golf activities or reliance on therapy services accessed through our platform. All services are provided “as is” without warranty of any kind.
10. Modifications
We may update this Agreement from time to time. If we make material changes, we will notify you via email or your member portal. Continued use of the service constitutes acceptance of the revised terms.
